Physician Case Manager (General Practitioner) – Part Time Remote

About the role

The Physician Case Manager (PCM) plays a vital role in remotely coordinating and managing the medical care of patients. This position uses telecommunication technology to provide virtual consultations, collaborate with healthcare teams and ensure seamless, patient‑centered care.

Key responsibilities

• Manage and follow up cases referred by Teladoc Health.
• Serve as the primary point of contact for patients, providers and other stakeholders, ensuring effective communication and coordination.
• Call patients, collect full medical records, and understand their medical status and enquiries.
• Conduct comprehensive assessments of patients’ conditions, symptoms, treatment history and review documents for Medical Experts.
• Collaborate with patients to develop personalized care plans based on their unique needs and resources.
• Complete clinical case summaries according to Teladoc standards and explain Expert’s Opinion Reports to patients.
• Continuously monitor patient progress, adjust care plans as needed, and provide medical guidance and education.
• Fulfil tasks within standard turnaround times and cooperate with the Medical Director and Care Coordinator on quality improvement initiatives.
• Deliver remote advisory services such as Expert Medical Opinions, health advisory, chronic disease care, coaching/education and referrals.

Required profile

• Medical degree from an accredited university or institution.
• Registered with the Medical Council of Hong Kong with a good practicing record.
• Currently clinically active.
• Proficient in English and Cantonese; Mandarin proficiency is a competitive advantage.

Required skills

• Proficiency in utilizing telecommunication technology for virtual patient consultations.
• Experience with telemedicine platforms.
