Care Coordinator

About the role

The Care Coordinator at Bupa Global in Hong Kong ensures that customers receive evidence‑based diagnostics and appropriate care throughout their hospital journey. The role supports pre‑admission, inpatient stay and discharge, aiming for optimal health outcomes and efficient use of resources.

Key responsibilities

• Review non‑complex inpatient and day‑hospital admissions, triage needs and direct customers to contracted providers.
• Update patient management systems with accurate clinical information and coordinate with hospital bed managers.
• Educate customers and families about care pathways, procedures and follow‑up requirements.
• Participate in case review meetings, resolve issues affecting care progression and liaise with interdisciplinary teams.
• Monitor discharge planning, negotiate test and procedure scheduling, and ensure compliance with care plans.

Required profile

• Clinical or CS background preferred, with experience in bed management and discharge planning.
• Strong analytical abilities and decision‑making experience in a clinical setting.
• Excellent interpersonal, communication and organisational skills.
• Ability to work with both clinical and non‑clinical personnel across all organisational levels.
• Working knowledge of the healthcare system and resources.
