Project Officer – End-of-Life Community Care

About the role

The Hong Kong Society for Rehabilitation is seeking a dedicated Project Officer to support the JCECC “Life Rainbow” End‑of‑Life Community Care Project. This role focuses on delivering psychosocial‑spiritual care and practical assistance to persons with late‑stage chronic illness and their families, while coordinating project activities and reporting.

Key responsibilities

• Assist in psychosocial‑spiritual care activities such as wish‑fulfilment, legacy projects and family outings.
• Provide practical support including escort services, patient‑sitting, equipment loan, home‑making and occasional personal care.
• Coordinate evaluations, data entry and maintain project records.
• Prepare administrative and budget reports for the project.
• Organise public education events and develop resource materials.
• Perform other ad‑hoc duties as assigned.

Required profile

• Degree holder or above.
• Minimum 1 year experience in administrative support and coordination.
• Experience working with frail elderly or similar target groups.
• Knowledge of chronic disease management, vocational rehabilitation, mutual support, volunteer service and e‑service delivery.
• Strong interpersonal, communication and time‑management skills.
• Mature, self‑initiated, innovative and a good team player in a multidisciplinary environment.
• Fluent in both Chinese and English.
• Willingness to work on a contract basis, with renewal dependent on service needs and performance.
