Community Health Officer – Exercise & Health Management

About the role

Health In Action Limited is seeking a Community Health Officer specialized in health management to deliver exercise‑based programmes for weight management, physical function, and sarcopenia prevention. You will work directly with low‑income families, building trust and empowering them to take control of their health through practical, sustainable lifestyle changes.

Key responsibilities

• Conduct community health assessments, screenings and follow‑up visits.
• Support and co‑ordinate health workshops, cooking classes, exercise sessions and other community programmes.
• Maintain service records, statistics and reporting for monitoring outcomes.
• Collaborate with schools, restaurants, workplaces and local partners to promote healthy living.
• Engage working‑poor families, build rapport and motivate health‑ownership.
• Design, lead and tailor exercise programmes for weight management, healthy ageing and sarcopenia prevention.
• Develop and deliver accessible health education materials.

Required profile

• Passion for preventive health and community‑based service delivery.
• Experience working with disadvantaged or low‑income populations.
• Ability to translate professional knowledge into practical, culturally appropriate solutions.
• Strong communication skills with service users, caregivers and partners.
